New Client Information

WELCOME to Lake Union Veterinary Clinic!

We are excited to meet you and your furry family member(s).  To make your first visit with us go as smoothly as possible, we ask that you take a couple minutes to give us a little information — that way the focus of your visit can be on your pet, not on paperwork!

1) Please review our COVID-19 policy and guidelines

The lobby is open and you are welcome to join your pet for their appointment, as long as:

      • You are not currently sick / not showing signs of anything contagious (coughing, sneezing, etc.), and
      • You wear a mask while inside the building.  If you do not have a mask, we can provide one.

2) Complete our ‘New Client’ information sheet (HERE)

The form gives us information like the names of people authorized to seek treatment for your pet(s), contact information, and consent for treatment.


3) Please forward your pet’s medical records to us.

You can contact your previous veterinarian to have them email the appropriate notes directly to us ( In the event your pet does not have previous veterinary records, please send us any information you have (breeder records, shelter notes, rescue paperwork).


Given the important nature of this information, we ask that you complete these requests prior to your visit.  If you need any assistance, please do not hesitate to contact us.  Thanks!
